This unique package focuses on guiding you through the creation of a holistic survivorship care plan. Survivorship plans have become a standard of care recently, yet many treatment centers are not yet providing patients with them. Coaching sessions in this package will cover:
- reviewing the standard key components of a medical survivorship plan
- adding in goals that address not only staying current on medical follow ups but also adhering to wellness goals related to prevention and other quality of life goals
- breaking down goals into doable steps
- setting up accountability
- coaching on blocks or obstacles that come up
- designed dialogues to help enroll your medical team in your holistic plan
This program includes:
- a 1 hour foundation session
- 3 x 30 minute coaching sessions
- 3 lifeline emails to receive email coaching between sessions
- package of educational materials on survivorship care planning

What is coaching?
Coaching is a simple yet effective resource that provides the support and tools you need to reduce stress, address fears and create action plans to empower you so you don’t feeling like you’re just surviving in life.
The Coach helps the client to promote and sustain the Client’s personal wellbeing and to grow instead of being overwhelmed and feeling lost, particularly during times of major challenges and change. This powerful relationship allows the Client to shift their perspective from focusing on what’s not working to taking action to create more of what they want in their lives. I am a Certified Professional Coach (CPC) and an Energy Leadership Index Master Practitioner (ELI-MP). Please view my complete page of relevant training.
